Zulekha Hospital Sharjah successfully performed a complex laparoscopic pyeloplasty for a six-month-old infant diagnosed with severe hydronephrosis, highlighting the hospital’s expertise in managing challenging paediatric urological conditions.

The infant was incidentally diagnosed with an enlarged left kidney during antenatal screening and was closely monitored after birth. Follow-up ultrasound scans revealed progressively increasing swelling of the left kidney, prompting referral to the urology department for specialised care.

Although the child appeared clinically stable, the parents occasionally observed intermittent abdominal fullness. Repeated ultrasound imaging confirmed progressive left-sided hydronephrosis, indicating obstruction in the kidney’s drainage system. Further investigations, including a nuclear renal scan, revealed delayed drainage and reduced function of the affected kidney, confirming the need for timely surgical intervention.

Advanced surgical intervention

Given the child’s young age, the case required a coordinated multidisciplinary approach. While Dr Saurabh Singh, Specialist Urologist at Zulekha Hospital Sharjah performed a laparoscopic left pyeloplasty, a minimally invasive surgery designed to relieve the obstruction and preserve kidney function, the procedure was supported by Dr Nabil, Paediatric Surgeon, along with Dr Abhijeet, Paediatric Anaesthetist, and Dr Hassan, Paediatric Intensivist, ensuring comprehensive surgical and post-operative care. Performing laparoscopic surgery in infants younger than six months presents unique challenges due to limited operative space, specialised instruments, anaesthesia considerations, and careful postoperative monitoring.

Positive outcome

Despite the complexity of the case, the surgery was successfully completed. The infant recovered well and remained stable during the postoperative period, with follow-up assessments showing encouraging progress.

Speaking of the urgency of this case, Dr Saurabh said, “Similar corrective procedures are often performed after the age of two to three years. However, in this case, progressively worsening kidney function required earlier surgical intervention to prevent permanent kidney damage.”

Doctors emphasise that many congenital kidney abnormalities are now detected during antenatal scans. While not all children diagnosed before birth require surgery, regular postnatal follow-up is essential to monitor kidney health and ensure timely treatment when necessary.
